Patient Information Procedures ~
The Family Practice, Pediatrics, & Gynecology Division


Medical records are maintained in a confidential manner. Information regarding your diagnosis, medications, test results, and other necessary items will be made available to your referring physician. Worker's compensation, insurance, disability, rehabilitation, and others can obtain information only with a signed release from the patient. Hospital records and records from other physicians or labs may be helpful to your SNMMA physician. You may be asked to sign a release so that we may obtain these records.



HIPAA Privacy Notification for SNMMA Patients

The misuse of Personal Health Information (PHI) has been identified by our government as a national problem. The employees and physicians at Southern New Mexico Medical Association (SNMMA) have undergone special training so that we comply with the government rules and regulations regarding the Health Insurance Portability and Accountabilty Act (HIPPA).

It is the written policy of SNMMA to properly determine the appropariate use of PHI in accordance with the government regulations. The written policy gives specific instructions on how your information is to be treated and the penalty for any violation of this protected helath information. We want to assure all our patients that we have always maintained a high standard regarding your health information and will continue to do so.

We also value your input to this new process. You will be asked to sign a consent outlining the policy of SNMMA on the use of your medical information. In short, we only use your information to keep you and/or your health care providers informed of your ongoing medical issues and to complete the billing process for your claims.
